Forehead Lift — It Isn't Your Eyelids That Sagged, It's Your Eyebrows That Came Down
"My eyelids feel heavy and droopy. All I need is an upper blepharoplasty (eyelid incision), right?"
This is a phrase I hear multiple times a day in my consultation room. Yet when I carefully evaluate patients who visit with this concern, in a remarkably large number of cases, the real culprit is not the eyelids—it is the eyebrows that have descended.
When the true cause is the brow, what happens if you only cut the eyelids? While it may feel slightly lighter for a brief period, within a few years the eye contour often returns looking even heavier, thicker, and more unnatural. Today, from the perspective of a board-certified plastic surgeon, I want to explain why this happens and why the forehead lift is the true cornerstone of mid-life eye rejuvenation.
Why 'Eyelid Surgery' Alone Is Often Not Enough
As we age, it is never just the eyelid skin that droops. The entire forehead and eyebrow complex descends as a unified anatomical unit. If you perform an upper blepharoplasty or sub-brow lift alone while leaving brow ptosis unaddressed, the vertical distance between the eyes and eyebrows becomes significantly narrower.
As the brows sit tighter against the eyes, the facial expression turns stern, fierce, and crowded. Because the fundamental root cause—eyebrow descent—was left untouched, sagging inevitably resumes over time.

▲ With aging, the eyebrows descend before the eyelid skin, significantly narrowing the distance between the eyes and brows.
When Sagging Is Severe, Upper Blepharoplasty Alone Is a 'Double-Edged Sword'
Performing an upper blepharoplasty alone in the presence of severe brow descent can often do more harm than good.
To help the eyes open more easily, surgeons may excise a wide strip—sometimes well over 1 cm—of delicate, paper-thin eyelid skin. Once that thin skin near the lash margin is removed, the much thicker skin that naturally sits beneath the eyebrow is dragged downward to form the new double eyelid crease.
- The double eyelid fold looks thick, bulky, and chronically swollen (the "sausage" crease)
- The space between the eye and eyebrow is tightly compressed
- The overall facial expression becomes stern, heavy, and unnatural
While the eye may physically open, the natural harmony of the gaze is compromised. Furthermore, once that delicate, paper-thin eyelid skin is excised, it can never be regenerated—even with revision surgery.

▲ (Left) When upper blepharoplasty is performed alone under a low brow, delicate thin skin is excised and thick sub-brow skin folds into the crease, creating a bulky fold and compressed brow-to-eye distance. (Right) When thin eyelid skin is preserved, a crisp, natural crease is restored.
Forehead Lift: Restoring Sagging at Its 'Anatomical Origin'
The definitive solution is clear: return what has descended back to where it originally belonged.
An endoscopic forehead lift elevates the forehead and eyebrows back to their youthful anatomical position, resolving laxity right at its source. Once the brows are properly positioned:
- The heavy weight pressing down on the eyes vanishes, allowing the eyes to open effortlessly
- The youthful distance between the eyes and eyebrows is restored, creating a soft, refreshed expression
- Deep forehead furrows and glabellar frown lines are smoothed away simultaneously

▲ Once the eyebrows are returned to their natural position, the entire eye contour regains a comfortable, soft, and youthful harmony.
Have a High Forehead? Simultaneous 'Reduction & Lift' Solves Both
- Relatively high/wide forehead: A Forehead Reduction Lift simultaneously lowers the hairline while elevating the brow complex.
- Balanced forehead proportions: An endoscopic forehead lift alone rejuvenates the eye contour, smooths forehead creases, and softens the expression without changing hairline height.
The Ideal Approach Is 'Synergistic Combination'; If Choosing One, Choose Forehead Lift
In clinical practice, combining a forehead lift, sub-brow lift, and upper blepharoplasty according to individual anatomy yields the most refined outcome. However, if only a single procedure is chosen, an endoscopic forehead lift—which addresses the root cause rather than cutting away at the symptom—is consistently the choice with the least regret.
