Cheek Fat above the Nasolabial Fold

When descended cheek fat is prominent just above the nasolabial fold, it adds heaviness to the mid-face and ages the overall impression. Precise removal of this localised area alone can visibly clean up the frontal contour and contribute to a more youthful look.

Q.How does facial liposuction differ from body liposuction?
A.The goals and techniques are different. Facial liposuction uses fine cannulae to precisely remove small amounts of fat from areas such as the nasolabial cheeks, under-chin, and jawline — areas that have descended with age or are a personal concern — to give the face a smaller and more youthful impression. Body liposuction removes stubborn, localised fat from areas such as the upper arms, abdomen, and thighs that resist diet and exercise, sculpting the contour of the body. Although both share the name "liposuction," the goals, techniques, and recovery timelines are all distinct.
Q.How does facial liposuction contribute to a more youthful appearance?
A.Ageing progresses alongside sagging through a process of fat redistribution. Volume in the upper face — forehead, temples, and anterior cheeks — diminishes, while fat in the lower face — above the nasolabial folds, under the chin, and along the jawline — descends and accumulates. The result is the classic "inverted triangle of ageing," where the upper face deflates and the lower face heavies. Facial liposuction addresses this descended, accumulated fat to shorten and smooth the profile, contributing to a more youthful impression. Combining fat grafting to replenish the deflated upper zones at the same time can provide a comprehensive rejuvenating effect.
Q.Can facial liposuction alone create a V-shaped jawline?
A.When laxity is mild, liposuction alone is often sufficient to define the profile and restore a V-shaped contour. However, when there is significant sagging or laxity in the SMAS layer itself, liposuction alone has its limits, and combining a mini facelift, full facelift, or platysmaplasty is recommended. At your consultation, the volume of descended fat and the degree of SMAS and muscular laxity are assessed together to determine whether liposuction alone is adequate or whether a combined approach is needed.
Q.Will the fat return after body liposuction?
A.Liposuction reduces the actual number of fat cells in the treated area. Because removed cells do not regenerate, significant re-accumulation in the same location is uncommon. However, the size of the remaining fat cells can still change with dietary habits, so other areas of the body may enlarge if overall caloric intake is excessive. If remaining cells enlarge considerably, the refined contour may become less defined, which is why maintaining a reasonably stable weight and lifestyle is advisable.
Q.Will there be visible scarring?
A.For facial procedures, entry points of 2–3 mm are made in discreet locations — behind the ears, beneath the chin, or inside the mouth — that are not visible from the front. For body procedures, micro-incisions of approximately 5–7 mm are placed along the underwear line, inside the navel, or within axillary creases, where they are normally covered by clothing. These marks fade significantly over time and become barely noticeable.
Q.How long is the recovery period?
A.After facial liposuction, the bulk of the swelling resolves within three to five days and most patients can return to normal daily activities within one week; residual swelling and the final refined contour typically settle over two to four weeks. After body liposuction, mild muscle-ache-like discomfort lasts one to three days and is managed with prescribed pain relief. Wearing a compression garment is recommended for four to six weeks. Light desk work can generally be resumed within three to five days, and strenuous exercise from one month post-op.
Q.Is it a good idea to combine liposuction with fat grafting?
A.For facial procedures in particular, the combination is frequently recommended. Because ageing follows the "inverted triangle" pattern — deflation above and descent below — removing descended fat from the lower face via liposuction while replenishing the deflated upper zones with fat grafting in the same session can provide a comprehensive rejuvenating result. A single shared recovery is an added practical benefit, which is why this combination is routinely discussed at consultations.
Q.Can multiple areas be treated in a single session?
A.Yes. For the body, it is common to treat the upper arms, abdomen, and thighs simultaneously. For the face, addressing the nasolabial cheeks, double chin, and jawline together in one session is standard practice. That said, treating a greater number of areas extends both the operating time and the recovery period, so the decision about whether to address everything at once or in stages is made with consideration for the patient's overall health, schedule, and priorities.
Q.Are there any side effects or precautions to be aware of?
A.Common post-operative reactions — swelling, bruising, temporary numbness, and firmness — resolve naturally over time. Rare potential complications include asymmetry, surface irregularity, and infection; AVA monitors contour progress at scheduled follow-up visits to allow early management if needed. Key precautions: ① wear the compression garment for four to six weeks; ② refrain from alcohol, saunas, and strenuous exercise for one month; ③ avoid vigorous massage of the treated areas for one month post-op; ④ staying well hydrated helps support lymphatic circulation and recovery.
