Which Treatment Do I Actually Need?

Patients arrive asking for a named procedure. It is usually the wrong question. Identify which of four problems you have, and the procedure follows on its own.

Four Problems, Frequently Confused

Almost everything treated in aesthetic medicine falls into one of four categories. They look similar in a mirror and require different solutions.

  1. Descent — tissue has moved downward. Cheek falling over the nasolabial line, jowls, a softening jaw.
  2. Volume loss — tissue has deflated. Hollow temples, flat cheeks, a groove under the eye.
  3. Skin quality — the surface has changed. Pores, fine lines, crepe texture, loss of firmness.
  4. Pigment — colour. Melasma, sun damage, post-inflammatory marks.

Treating descent with volume, or a shadow with a pigment laser, are the two most expensive mistakes in this field — not because the treatments are costly, but because they do not address what is wrong.

Two Self-Checks That Narrow It Quickly

The lying-down test. Lie flat and look in a hand mirror. Whatever largely disappears was descent — gravity was holding it down. Whatever remains is volume loss, texture or pigment.

The changing-light test. Move under different lighting. If a dark area changes with the light, it is a shadow cast by topography — a structural problem. If the colour is constant regardless of light and posture, it is pigment.

These are orientation tools, not diagnoses. Skin thickness, bone support and muscle activity modify all of it, and that assessment requires examination.

Q. I want to look less tired. Where do I start?

Establish first whether the tiredness is a shadow under the eye (structural), overall descent, or skin quality. The under-eye guide covers the most common version of this question.

Q. Can I avoid surgery altogether?

For skin quality and volume, largely yes. For established descent, no — devices soften the appearance but do not reposition tissue. Postponing indefinitely with repeated device sessions is a legitimate choice, but it should be a knowing one.

Q. How do I know if a clinic is recommending too much?

Ask what problem each item on the plan addresses. If a proposed treatment cannot be tied to one of the four categories, it is worth questioning.
