"Aging is not a single change. It descends, deflates, and thins—three processes occurring simultaneously."
Three Mechanisms of Facial Aging
No single procedure can address aging, because aging is not a single event.
Choosing the right combination of treatments starts with understanding the three changes unfolding in the face at once.
※ Clinical definitions of 30 specialized medical terms such as SMAS, retaining ligaments, and deep fat compartments are available in the AVA Plastic Surgery Medical Glossary.
① Procedures Addressing Sagging (Surgical·Non-Surgical Comparison)
Important: Non-surgical treatments are not substitutes for a surgical facelift. Sagging at different depths calls for different procedures. Detailed comparison (FAQ Q20).
② Procedures Addressing Volume Loss
Autologous Fat Grafting — Your own fat is harvested, purified, and grafted into hollowed areas. Long-lasting once it takes (reported graft survival ranges from 30–70% depending on site, constitution, and individual)
Hyaluronic Acid Filler — Immediate volume restoration, 6–12 months duration. Varies by site and product
Surgical feasibility determined by systemic health (hypertension, diabetes, medications) and skin condition
Where surgery is feasible, a facelift remains the most effective option; where risk factors are present, a staged non-surgical plan takes its place
Smoking and medication evaluation are critical to decision-making
Common Misconceptions ─ Clinical Perspective
"Thread lifts cost 1/10 of facelifts, so if I get multiple thread lifts, won't I achieve facelift results?"
No. Thread lifts and surgical facelifts are different procedures with different indications. A thread lift tractions the superficial skin layer; it cannot reach deep descent of the SMAS, midcheek, or platysma. However many sessions you add, the deep layers stay where they are — and the scarring can blur the dissection planes, potentially making a future surgical facelift more difficult.
"HIFU (Ultherapy) reaches the SMAS layer—isn't that the same as a facelift?"
HIFU delivers thermal energy to SMAS depth to stimulate collagen regeneration. However, it cannot perform the retaining ligament release and tissue repositioning that surgical facelifts accomplish. Reaching the same depth does not mean producing the same effect.
"If I fill everything with filler, I won't look saggy, so won't I avoid needing a facelift?"
Volume loss and sagging are different changes. Fillers restore hollows, but descended SMAS and deep fat remain in place—marionette lines, jowls, and platysmal bands do not improve. Additionally, excessive filler accumulation may cause facial puffiness or nodules.
"I'm in my 50s, but facelift surgery seems too invasive and scary…"
A safe, staged approach is entirely possible. The early-to-mid 50s are generally considered the most suitable time to begin a facelift, but the decision is always yours. Bridging with non-surgical treatments for 1–2 years while you watch how things change is also a reasonable path.
Single-Surgeon System
AVA Plastic Surgery operates a single-surgeon system where Dr. Kim Seungsoo personally performs every surgery from first incision to final suture.
The same physician handles consultation, design, surgery, and follow-up care, maintaining continuity of medical information.
No subsequent patient schedule begins until the current patient's surgery is complete. Zero delegated (ghost) surgery.
Nowhere is that continuity worth more than in anti-aging, where care unfolds in stages over years.
