Comprehensive Anti-Aging Guide Gangnam

Medically Self-Reviewed 6 International Journal Publications Dr. Kim Seungsoo, Board-Certified Plastic Surgeon · Based on Mendelson·Jacono International Standards

"Aging is not a single change. It descends, deflates, and thins—three processes occurring simultaneously."

Three Mechanisms of Facial Aging

No single procedure can address aging, because aging is not a single event. Choosing the right combination of treatments starts with understanding the three changes unfolding in the face at once.

Important: Non-surgical treatments are not substitutes for a surgical facelift. Sagging at different depths calls for different procedures. Detailed comparison (FAQ Q20).

② Procedures Addressing Volume Loss

③ Procedures Addressing Skin Quality

Skin quality procedures are provided as an integrated program at AVA Skin Clinic.

Recommended Approach by Age·Stage (Decision Tree)

※ This guide reflects general trends; actual indications will vary based on individual patient evaluation during consultation.

35–44 yearsPrevention·Early Stage

Primary Changes: Expression lines, early pore and texture changes, partial volume loss beginning.

Recommended Approach:

  • Non-Surgical: Botox (preventive), skin boosters, light- and heat-based care (introducing HIFU and radiofrequency)
  • Volume: Partial filler (nasolabial folds, infraorbital, forehead)
  • Surgical: Generally not recommended (insufficient indication)

45–54 yearsIntermediate Stage — Begin Facelift Consideration

Primary Changes: Perceptible midcheek descent, deepening nasolabial folds, early jowls, concurrent eyelid ptosis.

Recommended Approach:

55–64 yearsPrime Window for Surgical Facelift

Primary Changes: Deep midcheek descent, evident jowls, platysmal bands, accumulated volume loss.

Recommended Approach:

  • Surgical: A Dual-Deep Plane Facelift offers the fundamental correction — this is the stage at which the limits of non-surgical options become clear
  • Volume: Simultaneous autologous fat grafting with facelift (temples, cheeks, forehead)
  • Skin: Texture and tone care after facelift recovery (laser, skin boosters)

65+ yearsSafety Assessment Takes Priority

Primary Changes: Deep sagging + reduced skin thickness + reduced blood supply.

Recommended Approach:

  • Surgical feasibility determined by systemic health (hypertension, diabetes, medications) and skin condition
  • Where surgery is feasible, a facelift remains the most effective option; where risk factors are present, a staged non-surgical plan takes its place
  • Smoking and medication evaluation are critical to decision-making

Common Misconceptions ─ Clinical Perspective

"Thread lifts cost 1/10 of facelifts, so if I get multiple thread lifts, won't I achieve facelift results?"

No. Thread lifts and surgical facelifts are different procedures with different indications. A thread lift tractions the superficial skin layer; it cannot reach deep descent of the SMAS, midcheek, or platysma. However many sessions you add, the deep layers stay where they are — and the scarring can blur the dissection planes, potentially making a future surgical facelift more difficult.

"HIFU (Ultherapy) reaches the SMAS layer—isn't that the same as a facelift?"

HIFU delivers thermal energy to SMAS depth to stimulate collagen regeneration. However, it cannot perform the retaining ligament release and tissue repositioning that surgical facelifts accomplish. Reaching the same depth does not mean producing the same effect.

"If I fill everything with filler, I won't look saggy, so won't I avoid needing a facelift?"

Volume loss and sagging are different changes. Fillers restore hollows, but descended SMAS and deep fat remain in place—marionette lines, jowls, and platysmal bands do not improve. Additionally, excessive filler accumulation may cause facial puffiness or nodules.

"I'm in my 50s, but facelift surgery seems too invasive and scary…"

A safe, staged approach is entirely possible. The early-to-mid 50s are generally considered the most suitable time to begin a facelift, but the decision is always yours. Bridging with non-surgical treatments for 1–2 years while you watch how things change is also a reasonable path.

Single-Surgeon System

AVA Plastic Surgery operates a single-surgeon system where Dr. Kim Seungsoo personally performs every surgery from first incision to final suture. The same physician handles consultation, design, surgery, and follow-up care, maintaining continuity of medical information. No subsequent patient schedule begins until the current patient's surgery is complete. Zero delegated (ghost) surgery. Nowhere is that continuity worth more than in anti-aging, where care unfolds in stages over years.
