Gangnam Anti-Aging Complete Guide

Medical Advertising Law §56 Reviewed 6 SCI Publications Led by Board-Certified Plastic Surgeon Dr. Kim Seungsoo · Based on Mendelson·Jacono International Standards

"Aging is not a single change. Sagging, volume loss, and thinning — three processes occur simultaneously."

Three Mechanisms of Facial Aging

Anti-aging cannot be resolved with a single procedure because aging is not a singular phenomenon. Understanding the three concurrent changes in the face is essential to determine the appropriate combination of procedures.

Recommendations by Age & Stage (Decision Tree)

※ This guide reflects general trends; actual indications are determined by patient evaluation during consultation.

35~44 yearsPrevention·Early Stage

Main Changes: Expression lines, early pore and texture changes, some volume loss beginning.

Recommended Approach:

  • Non-surgical: Botox (preventive), skin boosters, light/thermal care (starting HIFU·radiofrequency)
  • Volume: Partial filler (nasolabial, under-eye, forehead)
  • Surgical: Generally not recommended (insufficient indication)

45~54 yearsMid Stage — Consideration of Facelift Begins

Main Changes: Noticeable midcheek descent, deepening nasolabial folds, early jowls, concurrent eyelid ptosis.

Recommended Approach:

55~64 yearsPrimary Indication Range for Surgical Facelift

Main Changes: Deep midcheek ptosis, prominent jowls, platysmal bands, accumulated volume loss.

Recommended Approach:

  • Surgical: Dual-Deep Plane facelift as fundamental solution. Non-surgical approaches show clear limitations at this stage
  • Volume: Concurrent autologous fat grafting with facelift (temples, cheeks, forehead)
  • Skin: Texture and tone care after facelift recovery (lasers, skin boosters)

65+ yearsSafety Assessment Takes Priority

Main Changes: Deep sagging + reduced skin thickness + decreased blood circulation.

Recommended Approach:

  • Surgical feasibility determined by overall health (hypertension, diabetes, medications) and skin condition
  • If feasible, facelift is most effective, but if risk factors exist, staged non-surgical approach
  • Smoking and medication evaluation are key to decision-making

Common Misconceptions ─ Clinical Perspective

"If a thread lift is 1/10th the cost of a facelift, wouldn't multiple thread lifts produce facelift results?"

No. Thread lifts and surgical facelifts are different procedures for different indications. Thread lifts involve superficial skin traction and cannot address deep sagging of the SMAS, midcheek, or platysma. Multiple sessions do not resolve deep-layer descent and may blur dissection planes, potentially complicating future surgical facelifts.

"Doesn't HIFU (Ultherapy) reach the SMAS layer, so isn't it the same as a facelift?"

HIFU delivers thermal energy to the SMAS depth to stimulate collagen regeneration. However, it cannot perform retaining ligament release and tissue repositioning that surgical facelifts address. Reaching the same depth does not equate to producing the same effect.

"If I fill everything with filler, I won't look saggy, so don't I not need a facelift?"

Volume loss and sagging are distinct changes. Fillers augment hollows but do not address descended SMAS and deep fat, leaving the corners of the mouth, jowls, and platysmal bands unchanged. Additionally, excessive filler accumulation can create a puffy appearance or nodules.

"I'm in my 50s, but a facelift seems like too major surgery and scares me…"

A safe staged approach is also possible. The early-to-mid 50s are considered the most suitable time to begin facelifts, but the patient's decision is paramount. Delaying with non-surgical methods for 1~2 years while observing changes before deciding is also a rational approach.

Single-Surgeon Responsibility (Single-Surgeon System)

AVA Plastic Surgery operates a single-surgeon responsibility system where only Dr. Kim Seungsoo enters the operating room. The same physician handles consultation, design, surgery, and follow-up care to maintain continuity of medical information. The next patient's schedule does not begin until the current patient's surgery is complete. Zero proxy surgery. The value of this system is greatest in areas like anti-aging that require staged, long-term care.
